Building Community Partners

 Bridge healthcare case managers and social workers with community partner agencies to improve coordination of patient care and decrease barriers for patient discharge.  

Bucks-Mont Community Advocacy Network Mental Health Advocacy Committee

Shares community and policy updates that impact the social sector in Bucks and Montgomery Counties, bringing these updates to members and the larger community for collective voice and action.

PA-Navigate (FindHelp) Committee

Key PA-Navigate stakeholders convene to discuss best practices and improve coordination of care for clients between county health systems and community benefit organizations to address social determinants of health.

Advance Care Planning Task Force

Hospital and community representatives working collaboratively reviewing policies and process for incorporation for Advance Care Planning including promotion of program, completion of forms, and distribution into electronic medical records.

Violence in Healthcare Task Force

Members of Bucks County District Attorney’s Office, local police departments, health systems, and community partners discuss best practices and resources to prevent violence in health care settings and support those who experience acts of violence.
